Occupational accident is a flat $146.00 per month, per driver — $143.00 premium plus $3.00 NITACA association dues. Your coverage is bound once we have both your signed form and the first month's premium.

Paying by card adds a 4% card processing fee — $5.84 on a $146.00 premium, for a card total of $151.84.
Refunds. If your form is not accepted, or the carrier denies your application, your premium is refunded — less the credit card processing fee, which our processor keeps. Pay by bank transfer, Zelle or check and there is no processing fee to deduct.
What happens next: we review it, submit it to the carrier the same day it reaches us during business hours, and coverage can be effective as soon as the next day. If anything on the form needs fixing we'll call you before it goes in — we'd rather catch it now than at a claim.

Owner-operator, or contract driver? If you own or lease the power unit, you're an owner-operator. If you drive someone else's truck and get a 1099, you're a contract driver — a different box. The form says it plainly: if the information you give makes you ineligible, premium is refunded and no claims are payable.
What are you pulling? Equipment sets your plan. Hopper bottom grain trailers qualify for Plans 1, 2 and 3. Tankers, dump, auto haulers, heavy machinery, and grain hauled in anything other than a hopper bottom are non-standard risk and can only go on Plan 1A. Checking a non-standard box while selecting Plan 1 is the most common error on this form.
